SUMMARY & BACKGROUND:
Since December 2016, the Toledo Fire and Rescue Department (TFRD) has partnered with ESO for emergency medical and fire incident reporting. Their product is currently integrated into our system and fulfills our software needs for state fire incident reporting, maintenance of medical records, and medical transport billing. Additionally, various local hospital systems utilize the ESO format to share patient outcome data. For these reasons, Fire requests the need to continue to use the ESO software.

The annual costs for this software over the term of the agreement are as follows:
Year 1 $79,184.00
Year 2 94,784.00
Year 3 94,784.00
Year 4 97,627.52
Year 5 100,556.35
Total $466,935.87

Summary
NOW, THEREFORE, Be it ordained by the Council of the City of Toledo:

SECTION 1. That the Mayor is authorized to enter a three-year software license agreement, including two additional one-year auto-renewal options, with ESO, Inc. for Electronic Patient Care & National Fire Incident Reporting software use upon terms and conditions deemed proper and requisite according to the Director of Fire and Rescue and the Director of Law.

SECTION 2. That, subject to annual appropriations, the expenditure of an amount not to exceed $466,935.87 over the term of the agreement authorized in Section 1 is authorized from the General Fund, Account Code 1001-53000-3341002SUPSRV for the use cost of ESO, Inc. software.
