SUMMARY & BACKGROUND:
Ohio Department of Health ("ODH") is in receipt of funding for lead hazard remediation through its contractual agreement with the Ohio Department of Medicaid ("ODM"). ODH is targeting Lucas County for primary prevention projects and desires to contract with the City of Toledo for lead hazard remediation on eligible residential properties located in the City of Toledo.

Through this contractual relationship, the City of Toledo Department of Neighborhoods will use these funds to remediate lead on up to 20 properties that have been reviewed and enrolled in the ODH/SCHIP lead program and referred to the Department of Neighborhoods by ODH. Services will include intake and enrollment of potential participants; pre-lead hazard control enrollment; post-lead hazard control notices; coordination and completion of paint inspections and risk assessments; coordination and completion of lead work specifications, construction costs, contractor assignments, clearance examinations and tenant relocation, as necessary.
