The City of Toledo's contract with FrontPath Health Coalition, the incumbent Preferred Provider Organization (PPO), expires on September 30, 2021. Both the Administration and vender agree to extend the current agreement through May 31, 2022.

Such agreement shall contain terms and conditions deemed proper and requisite according to the Director of Law and the Director of Human Resources.
