SUMMARY & BACKGROUND:
Ohio Department of Health ("ODH") is in receipt of funding for lead hazard remediation through its contractual agreement with the Ohio Department of Medicaid ("ODM"). ODM applied for Health Services Initiative funding through the State Children's Health Insurance Program ("SCHIP"). Phase 2 of this project requires ODH to conduct primary prevention in homes in six targeted counties. ODH has already initiated work in Summit County. ODH is now targeting Lucas County for the next primary prevention project and desires to contract with the City of Toledo for lead hazard remediation on properties located in Toledo's highest risk census tracts.

Through this contractual relationship, the City of Toledo Department of Neighborhoods will use these funds to remediate lead on 20 properties. Services will including intake and enrollment of potential participants; pre-lead hazard control enrollment; post-lead hazard control notices; coordination and completion of paint inspection and risk assessment; coordination and completion of lead work specifications, construction costs, clearance examinations and tenant relocation, as necessary.
