Recognize Neighborhood Health Association.
Summary
WHEREAS, Neighborhood Health Association of Toledo, founded in 1968 by members of the Black Community is being recognized by two national entities for the organization's positive work bringing low cost accessible healthcare to those in the Toledo area; and
WHEREAS, the American Heart Association awarded three achievement awards to NHA including the Target BP Gold + award jointly awarded by the American Medical Association, the Check. Change. Control. Gold Award, and the Target Type 2 Diabetes Participant award; and
WHEREAS, Holland Health Care and Navarre Park Family Health Center, two of Neighborhood Health Association's sites were recognized by the Million Hearts initiative of the Center for Disease Control and prevention and the Centers for Medicare and Medicaid as 2023 Champions; and
WHEREAS, the Million Hearts initiative recognizes Champions that demonstrate their commitment to help their patients with hypertension manage their blood pressure - achieving a hypertension control rate of at least 80%; and
WHEREAS, the recognitions from both the American Heart Association and the Million Hearts initiative confirm that NHA continues to meet the mission that "Through our exceptional health care services, we empower and educate, aggressively working to eliminate health care equities, while supporting personal responsibility for one's own health regardless of the ability to pay"; and
WHEREAS, the vision of Neighborhood Health Association ensures access to quality medical care for anyone in need. Through effective delivery of services, Neighborhood Health Association strives to become a national model of care for the under and uninsured in the community - empowering patients through responsiveness to their needs and fostering a workplace invigorated with enthusiasm for our singular purpose; NOW, THEREFORE,
