SUMMARY & BACKGROUND:
The City of Toledo's previous contract with Medical Mutual of Ohio, and will expire on July 31, 2021.

The City of Toledo's Department of Human Resources sought competitive requests for proposals in accordance with law and received three (3) proposals for stop loss insurance services related to hospitalization, surgical-medical, major medical, and medical management services for the City of Toledo's employees. The Human Resources Department reviewed each proposal, and it was determined that Anthem Life Insurance Company presented the lowest and best proposal. It is estimated that the cost associated with this agreement will be approximately $500,000.00 per year.

Summary
NOW THEREFORE, Be it ordained by Council of the City of Toledo:

SECTION 1. That the Mayor is authorized to extend the Agreement with Anthem Life Insurance Company to provide stop loss insurance services. Such Agreement shall contain terms and conditions deemed proper and requisite according to the Director of Law and Director of Human Resources.

SECTION 2. That expenditures for healthcare related third party services are authorized from Account Code 7083-17100-537410-1156001STDSTD. That said costs shall then be allocated to Account Code 517400 (Medical Insurance) within the various City organization units.

SECTION 3. That the Director of Finance is authorized to draw warrant or warrants against the account code in Section 2 in payment of the authorized obligations upon presentation of proper voucher or vouchers.
